Who is eligible for insurance?

All employees meeting the waiting period and working 32 or more hours per week.

What is the waiting period and when can my insurance go into effect?

The first of the month after you have completed six full months of employment. Example: If you were hired on January 15, you would be eligible for coverage beginning August 1.

What coverages are available?

The company offers group health, dental, life and accidental death as a package and they also offer the option of taking Short Term Disability and/or Long Term Disability. Colonial insurance offers additional optional coverage for cancer, accident, life, etc.

How will I be notified before eligible?

You will receive a packet during the month prior to your eligibility. You will then be contacted by a representative from Colonial to answer any questions you have and complete your enrollment. Even if you are declining coverage, it is important that you still reply with the necessary paperwork.

Do you offer anything for part-time employees?

Part-time are not eligible for the company group coverage or disability insurances. Colonial does offer a medical bridge policy that does have coverage for part-time employees.

On the medical insurance you are required to go to a network physician, what about dentists?

There are not any specific dentists that you are required to use.

Does the dental cover braces or implants?

No.

What is the co-pay for doctors?

Regular physicians and specialists are $25 co-pay.

What is the deductible for medical?

        $525 for plan year 2005.

Is there a drug card?

We have a card that allows for a discount when you purchase prescriptions. (It is not a co-pay card.) The receipt can then be filed against the deductible for the year and then you are reimbursed at 80%. We do have a mail order pharmacy that you can order name brand Rxs at $50 for a three month supply, or, generic at $25 for a three month supply, and the deductible is waived.

If I decline, elect coverage, etc., when can I make changes?

You can make changes during annual enrollment that will become effective on January 1. If you do not enroll in the STD/LTD at the time it is offered, you can apply at any time but will have to be approved and me be declined coverage. If there is a family status change, i.e. divorce, marriage, etc., you can make changes during the year with the supporting documentation.
